Patient Advocacy & Navigation

The Patient Navigator Program is a core HOYA Clinic initiative dedicated to promoting continuity of care and empowering patients to manage their health. Medical student navigators work longitudinally with patients across multiple clinical and community settings, providing individualized support tailored to each patient’s needs. Through regular check-ins, navigators help patients overcome barriers to care, better understand their health conditions, and build sustainable strategies for disease management.

Across programs including chronic disease management, wound care, street medicine, and refugee health, patient navigators assist with care coordination, accessing resources, and connecting patients to medical and social services. By combining education, advocacy, and ongoing support, the program aims to strengthen patient autonomy, improve health outcomes, and advance equitable, community-centered care.
